Enrollment Guidelines
 
Medication at School - If your child is required to take medication at school, a PERMISSION FOR MEDICATION ADMINISTRATION form must be completed before the school can give any medication.  Please request this form from the office.
 
Volunteer Applications - Any adult who wants to attend a field trip or help in the classroom or school is required to complete a volunteer application each year BEFORE volunteering can begin.  These forms are available in the office.

4-year old PreschoolEnrollment begins in January of each year.

Preschool provides children with opportunities to work and play, learn to share, accept and respect individual differences, learn to express him/herself, develop physical coordination, begin to work with language, literary and numeracy and general readiness for Kindergarten.

Skyline Vista has one preschool class in the morning and another in the afternoon; these classes meet four days per week as follows:

First day of school for Preschool for 2016-17 is Monday, August 22, 2016. 
 
Children must be four years old on or before October 1 to enroll.  To register for Preschool, we will need:
** Immunization record (will not be able to register without current immunizations / students in Preschool must have 4 DTP, 3 Polio, 3 Hepatitis B,  3/2/1 HIB, 4/3/2 PCV, 1 Varicella, and 1 MMR (unable to register without current immunizations)
** State-issued birth certificate (unable to register without a birth certificate)
** Proof of address (Acceptable proof: Cable bill, driver's license, electric/gas bill, insurance bill, lease, mortgage statement, phone bill, rent receipt, State ID, tax bill or water bill.)
** Physical (Physical must have been completed within the last year.  If the physical was done before August 2014, we will need a new physical before your child can start school in August 2015)

KindergartenEnrollment begins in January of each year.

Kindergarten provides children with opportunities to work and play, learn to share, accept and respect individual differences, learn to express him/herself, develop physical coordination, begin to work with language, literary and numeracy and general readiness for first grade.

During the 2015–2016 school year, Westminster Public Schools will continue to provide a full-day kindergarten program at each elementary school. Children must be five years old on or before October 1 to enroll.  There is no cost to parents for students attending this full-time kindergarten program. 

To register for Kindergarten, we will need:
** Immunization record (will not be able to register without current immunizations / students in Kindergarten must have 5 DTP, 4 Polio, 3 Hepatitis B, 2 Varicella and 2 MMR (unable to register without current immunizations)
** State-issued birth certificate (unable to register without a birth certificate)
** Proof of address (Acceptable proof: Cable bill, driver's license, electric/gas bill, insurance bill, lease, mortgage statement, phone bill, rent receipt, State ID, tax bill or water bill)

All new kindergarten students will be assessed with a screening instrument that has been developed by District staff. The assessment takes about ½ hour per child to administer and provides information about academic readiness in the areas of development, literacy, and mathematics. It can be administered in English or Spanish, depending on the child’s native language. When you come to Skyline to register your child in July, you will be able to select an assessment time on August 18 or 19.  The assessment will also be administered with each student at the completion of their kindergarten year as a measurement of growth in those areas.

The first day of school for Kindergarten for 2016-17 is Tuesday, August 16, 2016.
 
 
 Grades 1st through 5thTo register students in grades 1 through 5, we will need the following:
** Immunization record (students in grades 1 through 5 must have 5 DTP, 4 polio, 3 Hepatitis B, 2 Varicella and 2 MMR / unable to register without current immunizations)
** State-issued birth certificate (unable to register without a birth certificate)
** Proof of address (Acceptable proof: Cable bill, driver's license, electric/gas bill, insurance bill, lease, mortgage statement, phone bill, rent receipt, State ID, tax bill or water bill) 
** Attendance record from previous school
** Discipline record from previous school
** Special Education information (including but not limited to an IEP)
 
First day of school for grades 1st through 5th for 2016-17 is Tuesday, August 16, 2016.
