Immunization Requirements

Students must be compliant with the Colorado school immunization law in order to attend school. If your child has already received the required immunizations or has an exemption on file with the student’s school, please provide their immunization records/exemptions to the school office.
Requirements by Grade Level

Preschool
Colorado requires all preschool students to have a record of a current physical signed and dated by an approved health care professional. The following immunizations are required by state law, unless exempt, to attend school:
- 4 DTaP – Diphtheria, Tetanus, Pertussis
- 3 IPV- Polio
- 3/2/1 Hib – Haemophilus Influenza type B
- 1 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B (must be appropriately spaced with the 3rd vaccine given after six months of age)
- 1 Varicella – Chickenpox vaccine or history of disease provided by doctor/health provider
- PVC (Pneumococcal Conjugate) 4/3/2 doses are required, depending on the student’s current age and the age when the 1st dose was administered.

Grades K-5
Colorado State law requires all students entering Kindergarten to have the following immunizations, unless exempt, in order to attend school:
- 5 DTaP – Diphtheria, Tetanus, Pertussis (5 doses of DTaP vaccines are required unless the fourth dose was given on or after the fourth birthday, in which case only 4 doses are required.)
- 4 IPV – Polio (4 doses of Polio vaccine are required unless the third dose was given on or after the 4th birthday, in which case only 3 doses are required.)
- 2 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B (must be appropriately spaced with the 3rd vaccine given after 6 months of age)
- 2 Varicella –2 Chickenpox vaccines or history of disease provided by doctor/health provider

Grades 6-12 (Tdap required)
Colorado State law requires all students entering 6th through 12th grades to have the Tdap, unless exempt, for school attendance:
- 1 Tdap (Tetanus, diphtheria, pertussis) vaccine (6th-12th grade)
- 5 DTaP – Diphtheria, Tetanus, Pertussis (5 doses of DTaP vaccines are required unless the 4th dose was given on or after the 4th birthday, in which case only 4 doses are required.)
- 4 IPV – Polio (4 doses of Polio vaccine are required unless the 3rd dose was given on or after the 4th birthday, in which case only 3 doses are required.)
- 2 MMR – Measles, Mumps, Rubella
- 3 Hep B – Hepatitis B (must be appropriately spaced with the 3rd vaccine given after six months of age)

The summer break is a great time to get the required shots for the upcoming school year. Please make arrangements for your student at their health provider.

Kids First Health Care at Gregory Hills provides primary and preventative health care to infants, children and youth ages birth to 21. These services include physical and mental health care; immunizations and management of chronic health conditions such as asthma and weight management. For updated information on immunization clinics, visit kidsfirsthealthcare.org/gh or call 303-428-4384 for additional information. The following requirements apply only to the immunization clinic:
- $15 for patients without insurance
- No cost for patients with Medicaid
- Children must be at least four years old
- Must bring immunization record

Colorado law requires all students attending Colorado schools and licensed child care facilities to be vaccinated against certain diseases unless a medical or non-medical exemption is filed.
